-TANF Assessment fee waived (patient must be referred by DSHS)
-ADATSA Assessment fee waived (Patient needs referral from DSHS)
-DCFS Assessment fee waived (Patient must have referral from DCFS/CPS)

Requirements for Legal Assessment's

1. 5 Year Driving Abstract (must all legal assessments)
2. Breathalyzer Test Results (if possible)
3. Police Report (if possible)
4. Court Papers/Case Number (must have for all legal assessments)
5. I.D. (must have)
6.Social Security Number/card (must have)
7. Income Verification (pay stub, SSI check or unemployment check stub)
8.Medical Coupon/Insurance card (if applicable)
